This form in Adobe PDF can be downloaded on the link above and completed on your computer using Adobe Reader DC which is a free download from Adobe. Download the form to your computer SAVE IT TO YOUR COMPUTER then open it in Adobe DC click on Fill and Sign when you click on an entry line a Text Box will open allowing you to type in the information. Save the document using [File Save As] suggest you add your Lodge name and number to the file name (you can save the file to a folder on your computer if you want to keep it or save it to your Desktop and delete it after you have sent it) then attach it to an email and send to both email addresses noted on the form.
This form is in the new Word format and requires Word 2007 or newer to open. It is a fallible form which means you can fill it out on your computer save it and email it to the listed email addresses. To complete download the form and open it, first click on enable editing then click on the Title (Lodge, Number, Name etc.) a text box will open following the title clicked on begin typing the required information. Line lengths will adjust automatically for content.
